Completely fill out the application.

Only students with completed applications will be considered.  Follow the tips below to ensure your application is not delayed.

1. Mailing address
2. Social Security Number and if not a US citizen, provide Permanent Resident Number
3. Family Information: Number of people living in the household  
4. Family Information: Parent Statement of Financial Information
5. Family Information: Has either parent/guardian received a four-year degree from a college or university
6. 200 or more word essay (Mandatory)
7. Student and Parent Signatures

Step 3:
Turn in completed ETS application to your school counselor, college connections counselor, by e-mail, fax or mail them to:
