HealthSelect provides in-area network, in-area non/network, and out-of-area health care for employees and their dependents.
In-Area Coverage
The employee selects a primary care physician from a special list of doctors who have agreed to participate in HealthSelect. Employees may select a different primary care doctor for each covered dependent.
Other Referrals
You must go through your Primary Care Physician who will coordinate your medical care. He will refer you to specialists and for testing when appropriate.
Out-of-Network Benefits
If you decide to use the out of network benefits, your claims will be paid at 70% of reasonable and customary after a $1,500 deductible per family. You will need to file the claim forms.
