The forms below are used to apply for GI Bill benefits.
Please complete those forms that you need and return them to the
System Office Veteran Affairs or the financial aid office in order to apply.

All students must read and sign this form upon initial enrollment. You will only need to complete this form once, unless it has been over 1 year since your last attendance with Lone Star College System.

If you are a new student, you will need to provide one of the documents below that pertain to you along with the forms above:
- New student veteran must provide a copy of your DD214 or DD2384, whichever applies.
- New student dependent must provide a copy of your birth certificate or marriage license, whichever applies.

Clock Hour Request Form
This form is used ONLY for those attending a practical training or clinical requiring clock hour attendance per week for their degree. You may complete this form along with the required signatures and return it to the District Veterans Affairs office for certification.
