Application and Testing Information
Please use these links for general admissions information on the Basic Track and Transition to Professional Nursing - LVN/Paramedic to RN Articulation - Programs.Basic Track Applicants
1. Read the Basic Track ADN/RN Admissions Criteria Information Sheet.
2. Read and print HESI Test Ticket.
3. Complete Online Nursing Application (available January and February only and due March 7th by 4pm). It is advised that you start the online nursing application prior to taking the HESI entrance exam (if test is taken during the application period). You are able to log in and out of the application as needed. Otherwise, contact Nursing Advisor if scores are not present after 4-5 business days of having started online nursing application.
4. Turn in printed copy of the online nursing application, score sheet, LSC student evaluation or advising profile, evidence of Hepatitis B vaccination(s) (minimum first shot completed by application deadline), and FERPA form (available at http://www.lonestar.edu/14570.htm) by 4pm on March 7th. Do not turn in a blank score sheet with any zeros. The score sheet should display your points for your GPA, credit hours completed, HESI results and a total score.
**Please be advised that since Nursing Advisor commutes between the Health Professions Building and North Harris main campus, it is encouraged that you contact advisor at Juanita.Garcia@lonestar.edu prior to submitting application and supplemental materials in order to ensure you are able to submit paperwork in person.
**NOTE: You will not be able to complete your online nursing application until all of your transcripts
have been received, evaluated, and posted to your LSC student account by the Admissions Department. Please allow 4-6 weeks for transcripts to be received, evaluated, and posted to your account. Please allow 4-5 business days (after testing at a LSC Assessment Center) for HESI scores to be posted to your nursing application online.
If HESI test is taken outside of the LSC System, you will need to contact Elsevier directly to request an official score report be sent by mail to the Nursing Department at LSC - North Harris or electronically to Juanita.Garcia@lonestar.edu. Receipt of official score reports from Elsevier typically takes up to two weeks after request has been made.
LVN/Paramedic to RN (Articulation/Transition to Professional Nursing) Applicants
1. Read the Articulation/Transition Track (for current LVNs and Paramedics) ADN/RN Admissions Criteria Information Sheet.
2. Read and print HESI Test Ticket.
3. Complete Online Nursing Application (available mid July-early September only and due September 6th by 4pm). It is advised that you start the online nursing application prior to taking the HESI entrance exam (if test is taken during the application period). You are able to log in and out of the application as needed. Otherwise, contact Nursing Advisor if scores are not present after 4-5 business days of having started online nursing application.
4. Read and print the Transition Program for Articulating Students Checklist available at http://www.lonestar.edu/nursing-requirements.htm.
5. Turn in printed copy of the online nursing application and score sheet as well as the Transition Program for Articulating Students Checklist and all supporting documents by 4pm on September 6th. Do not turn in a blank score sheet with any zeros. The score sheet should display your points for your GPA, credit hours completed, HESI results and a total score.
