Enrollment Procedures

To enroll your child at the LSC-Montgomery Child Care Center, parents will need to submit the following completed information prior to the first day of attendance. Please be advised that the enrollment packet must be notarized by a licensed Notary of the State of Texas.

* Child Care Interest Form

* Child Care Enrollment Record which includes the following:

- Registration and Enrollment Record form **

- Parent Handbook Acknowledgement form **

- Medical Information Record including current shot records

- Teacher Information sheet

- School Policy form

- Notary Signature

- Current picture of your child

- Current picture(s) of parents or guardians, for safety purposes.

- A copy of parent's current registration statment (only for students, not staff)
