If a transcript of your tax return is requested, download the IRS Request for Transcript of Tax Return form (PDF).  After completing this form mail or fax it to the proper address as listed on the second page.  Once you receive the transcript from the IRS sign and date your signature before submitting  it to the financial aid office.
