This form is for LSC-Tomball faculty, staff, students, and administrators who want to request video recording, editing and/or distribution of video or other multimedia resources:

1. For use in LSC-Tomball courses (online and classroom-based) and/or;
2. In support of LSC-Tomball programs and services and/or;
3. To promote LSC-Tomball programs and services.

Please Note: You will need to contact OTS A/V for any computer and/or software setup, installation, or troubleshooting in a classroom environment.
