• A veteran (ch. 30 & 1606) that has never applied for GI Bill benefits before will need VA form: 22-1990
• A veteran (ch. 30 & 1606) that has received GI Bill benefits before, and is transferring to our school, and/or is changing degree plans needs form: 22-1995
• A dependent (ch. 35) that has never applied for GI Bill benefits before will need VA form: 22-5490
• A dependent (ch. 35) that has received GI Bill benefits before, and is transferring to our school, and/or is changing degree plans needs form: 22-5495

The forms below are used to apply for GI Bill benefits.
Please complete those forms that you need and return them to the
System Office Veteran Affairs or the financial aid office in order to apply.

Academic Evaluation Request
All students must complete this form when you are new to our school, or you are changing your degree plan.

Student Responsibility Form
All students must read and sign this form upon initial enrollment. You will only need to complete this form once, unless it has been over 1 year since your last attendance with Lone Star College System.

In addition:
If you are a new student, you will need to provide one of the documents below that pertain to you along with the forms above:

• New student veteran must provide a copy of your DD214 or DD2384, whichever applies.
• New student dependent must provide a copy of your birth certificate or marriage license, whichever applies.

Additional Forms

Clock Hour Request Form
This form is used ONLY for those attending a practical training or clinical requiring clock hour attendance per week for their degree. You may complete this form along with the required signatures and return it to the District Veterans Affairs office for certification.
