The purpose of Incident Management is to identify the avenues that may be used to make a report of suspected or actual wrongful conduct, depending on what works best for the circumstances. Incident Management manages the process for LSC’s incident reporting system by analyzing each report for referral to the appropriate LSC official in accordance with Lone Star College (LSC) Board Policy and Chancellor's Procedures.

LSC Incident Reporting System

The LSC Incident Reporting System is available for you to submit good faith inquiries, concerns, incident reports, or complaints regarding any type of alleged inappropriate conduct or non-compliance with federal, state, or local laws LSC Board Policies, and Chancellor’s Procedures. This includes concerns of sexual misconduct (sexual harassment, sexual assault, Title IX). Reports may be made anonymously. While it is your decision to be anonymous, please understand this may limit the College’s ability to assist you. If you are uncertain about reporting, please consult with college leadership to obtain assistance.

Step 1: Access the System

• Access the system for a web-report LSC Incident Reporting System
• Leave a message regarding your concern by dialing direct 832-813-6705

Step 2: Complete a Report

• Follow on-screen instructions.
• Complete the report, providing detailed information:
  • When did the incident occur?
  • Where was the location of the incident?
  • What are the specific details of the incident?
  • Who was involved in the incident?

Step 3: Write Down Case Number, Username, and Password

• Create a username and password. Retain the Case Number, Username, and Password in a safe place. You will need them to follow up, review, or amend your report.
