Dual Credit Forms

To get started successfully in this process, please review the following, complete, and return the required information.

High School / Private School / iSchool Packet

The following are required.

  1. Course Approval, Parental Consent, Waiver for Dual Credit Registration Form
  2. Bacterial Meningitis Vaccination Requirement (Not required).
  3. FERPA (not required but recommended)
  4. Drop Form (only necessary when a student needs to drop a class).

Homeschool Packet

The following are required.

  1. Complete the Dual Credit Exception Admissions and Parental Consent Form 
  2. Complete the Homeschool Agreement Partnership Form
  3. Homeschool Graduation Plan Template (must be typed and signed by Homeschool Admin)
  4. Homeschool Transcript Requirement Checklist (Please use this as a guide if you are using your own format. All Transcripts must be typed and signed by Homeschool Admin - Homeschool Transcript Template (If you wish to use our format) All Transcripts must be typed and signed by Homeschool Admin.
