Student Organization Resources

LSC-Montgomery RSO Annual Registration for Clubs and Organizations
This form allows returning organizations to register for the 26-27 school year. Deadline for submission for Fall 26 is October 15th. Not completing this form will result in suspension of club funds.

LSC-Montgomery New RSO Interest Form
This form allows students to submit information to form a new RSO for the 26-27 school year.

LSC-Montgomery RSO Advisor Agreement
This form is to be completed by ALL RSO advisors no later than October 15th for the Fall 26 semester. Not completing this form will result in suspension of club funds.

LSC-Montgomery RSO Officer Agreement
This form is to be completed by ALL RSO officers no later than October 15th for the Fall 26 semester. Not completing this form will result in suspension of club funds.

LSC-Montgomery Facility and Equipment Use Agreement
This form is to be completed by ALL RSO officers and advisors no later than October 15th for the Fall 26 semester. Not completing this form will result in suspension of club funds.

LSC-Montgomery RSO Membership Roster Form
The membership roster form is open from September 15th-October 15th for Fall 26. Only LSC students should be included on the roster. Student names and ID numbers are required. The roster should ONLY contain those two columns.

LSC-Montgomery RSO Meeting Attendance Form
Please use this Formstack to submit your organizations attendance files for meetings and events. Please only upload one roster at a time on an Excel sheet with the students' 7-digit ID number in column A only. If you upload more than one roster at a time, we will contact you and kindly request that you submit them all individually for our records.

Please utilize the Club and Organization Request Form to submit a request. Student Life will work with our MarCom department if you are in need of any of the following:

Club and Organization Materials

Sample Constitution
This document can be customized for each organization. The shaded areas should be changed to reflect the requirements and responsibilities as identified by the members of the organization.

Event/Fundraising Request
Any clubs or organizations wishing to hold a fundraiser or event must submit this form at least two weeks prior to the event date indicated. The Office of Student Life will need to give prior approval before an event can take place.

  • Deposit Memo Used at the Business Office to deposit funds from fundraisers. Requires the Program Manager's signature.

Vendors and Performers

Contracts and agreements for vendors and performers. Organizations that plan to have a vendor or performer on campus for an activity or program must complete the necessary paperwork found below. Please note that this process generally takes 6 to 8 weeks.

Process

  1. Have vendor sign our contract, produce an invoice, and complete a Vendor Application (if LSCS has not used this vendor previously)
  2. Bring signed contract and invoice to the Office of Student Life
  3. The Office of Student Life will handle all signature requests and will request a check.
  4. Once the contract is returned, scan a copy to the vendor

Speakers

Per the LSC policy regarding invited speakers, student organizations, registered or not, and or System faculty may invite speakers to speak on System premises, subject to the System’s Viewpoint- and Content-Neutral approval. The System must approve the speaker before an invitation is sent. The approval request must be made in writing to the appropriate college president or the college president’s designee. A speaker cannot be disinvited after an approved invitation has been sent unless the speaker or event creates physical safety and or Viewpoint- and Content-Neutral logistical concerns and the General Counsel approves the invitation revocation. The invitation ends once a speaker declines orally or in writing. Notwithstanding the above, speakers may not be invited to speak during the last two weeks of a semester.

All invited speakers must be submitted via the link below for approval from the Office of the VPSS.

 


Travel

  1. Confirm that budgeted funds are available for the trip, including the leasing of vehicles suitable for the trip.
  2. Be familiar with the process used to respond to any special needs of a student with a disability
  3. Submit the Request for Approval of College Sponsored Travel and the College Sponsored Travel paperwork to the Program Manager for Student Life four weeks prior to travel.
  4. The advisor or designated college official must accompany the student organization to events in which the student organization represents Lone Star College.

After receiving approval for the trip, the advisor must advise all students of the trip and take the following additional steps prior to departure.

*In the event that a student experiences a medical issue during approved student travel, please complete a service now claim with the Office of Risk Management. Be sure to contact your immediate supervisor so they are aware of this incident. Fill out the description section and attach the Travel Form 5 (TR 5 Incident Report) to the student injury claim. Submitting a claim must be completed even if the student does not seek or require medical attention.

Separately, if a student, faculty, or staff member is injured or experiences a medical issue while on campus, please complete the Risk Management Incident Report via service now.

*Please note: Include a photo image of your driver's license when completing TR4.

3. Advisor should bring all TR4 forms and emergency contact information when traveling. If you are renting a vehicle or staying in a Texas hotel, please visit Employee Intranet > Forms and Resources > Administration and Finance > Tax to download the appropriate paperwork.
